W poszukiwaniu czynników transdiagnostycznych dla anoreksji i zaburzeń ze spektrum autyzmu u kobiet w normie intelektualnej – krótki przegląd literatury

W ostatnim czasie nastąpił wzrost zainteresowania problematyką istnienia czynników transdiagnostycznych, które miałyby pomóc w wyjaśnieniu etiologii i mechanizmów podtrzymania różnych zaburzeń psychicznych. Zaburzenia odżywiania (EDs – eating disorders) to wieloczynnikowa kategoria objawów związanych z dysfunkcyjnymi myślami, zachowaniami oraz emocjami wobec wyglądu oraz odżywiania się/jedzenia. EDs charakteryzują się przewlekłym przebiegiem i częstymi nawrotami, co motywuje do poszukiwania zmiennych istotnych dla rozwoju i podtrzymania tych objawów. W XX wieku zauważono, że EDs mogą być powiązane z rozpoznaniem zaburzeń ze spektrum autyzmu (ASD – autism spectrum disorder). W literaturze przedmiotu trwa dyskusja o tym czy EDs mogą być manifestacją ASD. W niniejszym artykule przeprowadzono szybki przegląd badań, którego celem była próba odpowiedzi na pytanie o zależności między EDs a ASD. Przeanalizowano doniesienia z badań czynników behawioralnych i neuronalnych wspólnych dla tych zaburzeń, uwzględniono również wnioski z systematycznych przeglądów badań. Potwierdzono obecność cech typowych dla ASD w populacji kobiet z rozpoznaniem EDs, a szczególnie anoreksji psychicznej (AN – anorexia nervosa). Podobieństwo dotyczy funkcjonowania społecznego, deficytów wykonawczych oraz obecności stereotypowych zachowań. Zaobserwowano strukturalne i funkcjonalne zmiany w ośrodkowym układzie nerwowym, które są istotne dla obszarów stanowiących deficyty w ASD i AN. Wnioski z badań mogą przyczynić się do lepszego projektowania interwencji terapeutycznych dla wybranych zaburzeń.
